Existing NHS patient claiming free NHS dental treatment will be required to provide proof of exemption when arriving at the practice, BEFORE visiting the dentist. If proof cannot be provided patients will be charged for their appointment but will be able to claim a refund from the NHS by completing an NHS refund form HC5(D).
